How does it work?
Enter the patient’s information below (choose the state where they will be for treatment, even if it’s not where they live).
Select an appointment time that works for you.
During registration, indicate if the patient is enrolled in Medicaid. If not, select “Yes” to “I would like to request financial assistance”.
Choose the amount of financial assistance you need - 25%, 50% or 75% off of the cost of visits for the first year (as funding allows). Medicaid patients will automatically be registered for 100% financial assistance.
Complete the registration process.
